Thumb-base arthritis is common

Osteoarthritis at the base of the thumb affects a large share of adults over fifty, women more than men, and it responds well to exercise and a splint.

Tendons adapt slowly

Tendon remodels over months, not weeks. Pain improves long before the tissue is finished — which is why this runs thirty sessions.

The pain-monitoring model

Training within tolerable pain that settles by the next morning gives better outcomes than avoiding pain entirely.

A

Resist

Resisted thumb extension + abduction reps
A1

Resisted thumb extension

60 s · slow reps

Other hand resisting the back of the thumb; straighten it out against the resistance.

Easier gentler resistance  ·  Harder more resistance

R1R2—
A2

Resisted thumb abduction

15 reps · ×2

Thumb moving away from the palm against the other hand; three seconds back each rep.

Easier gentler resistance  ·  Harder more resistance

R1R2
B

Open

Web space opener + pinch endurance
B1

Web space opener

60 s · slow reps

Open the space between thumb and index gently with the other hand, then release.

Easier smaller range  ·  Harder hold three seconds

R1R2—
B2

Pinch endurance hold

4 × 20 s · ×2

Pinch and hold for twenty seconds; the thumb joint must not collapse inward.

Easier shorter holds  ·  Harder firmer object

R1R2
C

Endure

Finger extension endurance + grip-release speed
C1

Finger extension endurance

60 s · slow reps

Open the hand against the band repeatedly for the full minute — count clean opens.

Easier no band  ·  Harder thicker band

R1R2—
C2

Grip-release speed

20 reps · ×2

How to run it

  1. The thumb is forty per cent of hand function. It gets its own phase.
  2. Thumb-base arthritis loves the muscle beside the index finger — that one is trained every session here.
  3. Pinch is trained in a stable position: the thumb pad presses, the joint does not collapse.
  4. Dexterity is a skill. Practise it slowly and accurately before practising it fast.
